@dsmask80 : Thank You. Me too got pam_listfile(vsftpd:auth): Refused user root for service vsftpd in the log. Looks like lftp caches the username and password, and gives a formal sub-prompt, which made me misjudge that an actual connection is established. It tries to make the actual connection only when you enter a valid command. I checked the time when I enter the command and it matches the refusal time in the log.

Thank you. If simply does not apply is what happens to stuff after the except directive, then perhaps I was assuming an implicit "default deny". Also the following clause from man 5 hosts_access re-iterates your answer. EXCEPT Intended use is of the form: ‘list_1 EXCEPT list_2´; this construct matches anything that matches list_1 unless it matches list_2.
